Carolina herself said that he died on the Tartarus, that the ship crashed. Maybe he wasn't pulled away from his timeline before dying, maybe he did die and simply didn't notice. He's never believed in heaven or hell, but he can see all of this being a punishment. Especially since the Director abandoned him and didn't come back.
